As you know, providing a safe, fun, and inviting experience for our guests is of utmost importance to us. We cannot always be in the building, and we cannot always see everything, so we appreciate your eyes, ears, and honest feedback. We are hoping you observe our staff doing things right, but when they are not, this is a valuable tool to help educate, coach, and grow.

Please do not let the manager or staff know you are here to observe or that you are a part of our Guest focus group family.  Please take notes and fill the form out within 48 hours (about 2 days) of your visit – reimbursement are not to exceed 100.00 and rounded to the nearest dollar amount.  If you do not want reimbursement, please just note it on this form. Please do not be stressed or feel obligated to answer every question, but please know the more informed we are, the more we can grow and better ourselves and our team.
