As you know, providing a safe, fun, and inviting experience for our guests is of utmost importance to us. We cannot always be in the building, and we cannot always see everything, so we appreciate your eyes, ears, and honest feedback. We are hoping you observe our staff doing things right, but when they are not, this is a valuable tool to help educate, coach, and grow.

•Please do not let the manager or staff know you are here to observe or that you are a part of our Guest focus group family
•Please fill the form out within 48 hours (2 days) of your visit
•Meal reimbursement available up to $150 with itemized receipt; processed within 48 hours of completion
•Please attach all required photos and documentation
